Crisis Support & Stabilization

This short-term service is designed for families facing a sudden medical, behavioral, or care-related crisis. We act quickly to assess the situation, stabilize what’s urgent, and put a plan in motion, providing both hands-on support and access to trusted resources.

Hospital or Facility Discharge Planning
Seamless, hands-on coordination for a safe and supported return home.
Transitions from inpatient settings, whether medical, rehab, or psychiatric, are often complex and high-stakes. This service ensures nothing is missed. We coordinate with hospital teams, secure resources, and provide in-person support through every step of the discharge and adjustment process.

What to Expect When You Work With Us
Families often come to us feeling overwhelmed—unsure where to start, who to trust, or what the next step should be. Our role is to bring clarity and calm to the process. Here’s what you can expect when working with Northern Care Advisors:

We are a private-pay firm and do not bill insurance. Our services are billed hourly, and we provide detailed estimates before beginning any engagement.

Invoices are sent monthly between the 1st and the 5th.

Our preferred payment methods are eCheck and ACH. Credit cards are also accepted with a processing fee, and you’re welcome to mail a paper check if you prefer.
You can make a payment quickly and securely through our online payment portal. Just click the link, follow the prompts, and enter your payment details—it’s that simple!

Becoming a client is a simple, collaborative process. After we connect for a Discovery Call and confirm that our services are a good fit, onboarding begins:
The payer signs the Services Agreement.
The client (or their representative) signs the consent forms.
That’s it! Once onboarding is complete, we’ll get you scheduled—and the real work (and momentum) begins.

We offer regular updates via phone, email, or video, depending on your preference. Many of our clients have family members or guardians in other states. Communication is a core part of our service.
